How do I get and fill in the Attendance Allowance form?

For people over State Pension age, or their family, getting ready to claim Attendance Allowance.

The Attendance Allowance claim form is called AA1A. You can apply online, print the form, or ask for one by phone.

  • Call 0800 731 0122 for a paper form, Monday to Friday, 8am to 6pm.
  • If you call for a form and return it within 6 weeks, your claim starts from the date you called.
  • Post it to Freepost DWP Attendance Allowance. You do not need a stamp or postcode.
  • You get a text or letter within 3 weeks saying when to expect a decision.

What the form is

The Attendance Allowance claim form (code AA1A) is a 32-page form. DWP uses it to decide if you need help with personal care or supervision because of a disability or health condition. It comes with 12 pages of notes (AA1 notes) that explain each question.

You can claim if you have reached State Pension age and have needed help for at least 6 months. If you are terminally ill, different rules apply.

Tip: if you phone for a form and send it back within 6 weeks, your claim starts from the date you called. This can help if you need time to fill it in.

What you need

  • your National Insurance number
  • your address and contact details
  • details of your disability or health condition and medicines
  • your GP surgery's name and address, and any other health workers
  • dates of any stays in a care home, hospital or hospice

If you are not a British or Irish national, the notes ask you to send your passport, travel documents and any Home Office letters.

Tips for filling it in

  1. Use black ink. Do not worry about spelling or mistakes.
  2. Keep a short diary first. Write down what you needed help with, or found hard, over one or two days.
  3. Cover a whole day and night. Start when you get up, and go through 24 hours to the next morning.
  4. Describe a good day and a bad day if your condition changes.
  5. Send your diary if you want to. The notes say you can send it in with your form.

If you are terminally ill, ask your doctor or specialist for an SR1 form. It is free.

Sending it

Post the form to Freepost DWP Attendance Allowance. Do not write anything else on the envelope. You do not need a postcode or stamp.

After you send it

You get a text or letter within 3 weeks telling you when to expect a decision. Then you get a letter with the outcome. DWP may contact your GP about your condition. In 2026/27 Attendance Allowance is £76.70 a week (lower rate) or £114.60 (higher rate).

If you disagree with the decision, you can ask for a mandatory reconsideration within one month.

If you live in Scotland

Attendance Allowance is being replaced by Pension Age Disability Payment from Social Security Scotland. Apply for that instead.
